Successfully complete the game, then begin a new game in Close Combat mode. Finish the first two scenes of the first level. Exit to the main menu after the third scene begins. Select 'Unlocked Acts', and choose the final level. Select any character, and choose the same difficulty you used before. Ignore the message, and begin the level. Finish the entire last level to get the 'Butcher Would Be Proud' achievement. You can activate cheat codes, but must disable them during the final scene against Immortal.
