If not state otherwise below, press F1 at main menu.
Listen for 'Trainer Activated'.
Press desired hotkey. All hotkeys can be changed on the trainer.

Notes

This game is suffering from random crashes and has nothing to do with the trainer, try this tips to minimize that:
1.Enable the cheats once you're in the game then disable them before going back to the main menu.
2.Don't take too many round trips back to the main menu.
3.Don't pause and unpause the game rapidly.
4.Avoid using 'Set Inventory Items to 10' too much.
Unlimited Health:
This option simply gives you Unlimited Health and it doesn't saves you from losing limbs.
Set Inventory Items to 10:
This option set all of the items in your inventory to 10 and it's one shot only meaning each time you enable it,it turns itself off.
It's kinda unstable and might crash the game, to minimize this once in the game pause and unpause the game two times then enable it.
