You need the full version of Kingdom Hearts III, updated to the latest patch. It's best to have finished the base game before starting: the story of Re Mind takes place during the finale and spoils the surprises for those who haven't gotten there yet. To get started, choose the Story Scenario from the DLC menu.
Sora, without a body, travels back to the Keyblade Graveyard shortly before the final battle to save Kairi, and relives the personal clashes of the seven guardians of light one by one. It's a parallel story to the ending of the base game, not a sequel.
